How effective is changing type faces to indicate user input, user actions, window titles, interface items, and so on in user documentation?

I think this is very effective and use this in my documentation.

This is somewhat effective, but time-consuming. I use it.

I am unsure of the value, but my organization requires it, so I use it.

I am unsure of the value, and I don't use it.

Detracts from the documentation and makes it harder to use, not easier. I don't use it.

Absolutely valueless to my users, and they dislike it. I don't use it.

I have NO idea! What does everyone else think?

It might be interesting to consider the issues related to localization. How much work is implied when translating and re-formatting this kind of text?
